Meridian and MeridianComplete (Medicare-Medicaid Plan) Gas Mileage Reimbursement and Meals & Lodging Programs

Beginning May 1, 2024, SafeRide Health will be supporting the Meridian Health Plan Medicaid and MeridianComplete (Medicare-Medicaid Plan) population in Michigan with non-emergency medical transportation (NEMT) services. To ensure no interruptions for passengers and drivers utilizing the gas mileage reimbursement mode of transportation, please ensure drivers complete registration using the online application portal. If a ride is administered on or after May 1 without updated driver information on file, the driver will not be reimbursed.

Claims and Information


To be reimbursed, all claims need to be filled out using the Michigan Gas Mileage Reimbursement form (PDF opens in a new tab). Please complete the form and send back to SafeRide Health via email or U.S. Postal Services mail.

Meals and Lodging


To be reimbursed for a trip requiring meal(s) or lodging coverage, the Michigan Meals and Lodging Claim Form (PDF opens in a new tab) must be completed and submitted to SafeRide Health. Receipts are required for all meals and lodging reimbursements. Please review the form for additional instructions and details. For transportation of a minor (12 to 17 years old) without an escort, please have the legal guardian fill out the Michigan Medicaid Minor Consent Form (PDF opens in a new tab).
