Beginning April 1, 2025, SafeRide Health will be supporting the Community First Health Plans member population with non-emergency medical transportation (NEMT) services. To ensure no interruptions for passengers and drivers utilizing the individual transportation/gas mileage reimbursement mode of transportation, please ensure drivers complete registration using the online application portal. If a ride is administered on or after April 1st, 2025 without updated driver information on file, the driver will not be reimbursed.

Claims and Information
To be reimbursed, all claims need to be filled out using the Individual Transportation Program form (PDF opens in a new tab).
