Beginning November 1, 2024, SafeRide Health will be supporting the Sunflower Health Plan population in Kansas with non-emergency medical transportation (NEMT) services. To ensure no interruptions for passengers and drivers utilizing the gas mileage reimbursement mode of transportation, please ensure drivers complete registration using the online application portal. If a ride is administered on or after November 1 without updated driver information on file, the driver will not be reimbursed.

Claims and Information
To be reimbursed, all claims need to be filled out using the Kansas Gas Mileage Reimbursement form (PDF opens in a new tab). Please complete the form and send back to SafeRide Health via email or U.S. Postal Services mail.

Meals and Lodging
To be reimbursed for a trip requiring meal(s) or lodging coverage, the Kansas Meals and Lodging Claim Form (PDF opens in a new tab) must be completed and submitted to SafeRide Health. Receipts are required for all meals and lodging reimbursements. Please review the form for additional instructions and details. Additional Forms
The Level of Service Form (PDF) is to be completed by your primary care physicians or designees (physician assistant, nurse practitioner, or clinical nurse specialist). The form must be completed in its entirety.
